Hormone replacement therapy (HRT), and specifically bioidentical hormone replacement therapy (BHRT), is the medical treatment of hormone imbalances and deficiencies using hormones that are molecularly identical to those your body produces naturally. BHRT is used for both men and women to address symptoms of hormone decline, optimize hormonal balance, and support long-term health. This guide explains BHRT, when it's appropriate, what it costs, and how to find a qualified hormone therapy clinic.
BHRT uses hormones that are chemically identical to the hormones naturally produced by your body. The term 'bioidentical' distinguishes these from synthetic hormones, which have a different molecular structure. Bioidentical hormones are typically derived from plant sources (like soy or yams) and compounded by specialized pharmacies to match your individual needs.
For women, BHRT typically includes estradiol (the primary estrogen), progesterone, and sometimes testosterone. It's used to address symptoms of perimenopause and menopause — hot flashes, night sweats, mood changes, sleep disruption, vaginal dryness, and bone density loss — and increasingly for general hormone optimization in women with suboptimal levels.
For men, BHRT primarily involves testosterone replacement (covered in our TRT guide) but may also include other hormones like DHEA, and in some cases, estrogen management. The comprehensive approach to hormone optimization considers the interplay between all hormones, not just the one that's most obviously deficient.
For women, BHRT is most commonly considered during perimenopause and menopause, when natural hormone production declines. Symptoms that may indicate hormone imbalance include hot flashes, night sweats, mood changes, sleep problems, low energy, reduced libido, brain fog, and changes in weight or body composition. BHRT can be highly effective at addressing these symptoms.
For women not in menopause, BHRT may be appropriate if lab work shows suboptimal hormone levels and you're experiencing symptoms. Some women have hormone imbalances due to stress, thyroid issues, or other factors that BHRT can help address.
For men, hormone therapy is primarily about testosterone (see our TRT guide), but comprehensive hormone optimization may also address thyroid, DHEA, and other hormones. The best hormone clinics take a whole-system approach rather than treating hormones in isolation.
Hormone therapy is not appropriate for everyone. Certain conditions — including some cancers, history of blood clots, and active pregnancy — may contraindicate hormone therapy. A qualified clinic will screen for these during your consultation.
BHRT costs vary based on the hormones prescribed, the delivery method, and the clinic's model. Initial consultations range from $150 to $400. Comprehensive hormone panels run $200 to $500. Once established, monthly BHRT costs typically range from $200 to $400, which usually includes hormones, follow-up labs, and provider access.
Delivery method affects cost. Oral bioidentical hormones are typically the least expensive. Topical creams and gels are mid-range. Pellets (implanted under the skin and lasting 3–6 months) are more expensive per insertion but don't require daily administration. Your clinic will recommend the best delivery method based on your needs and preferences.
BHRT is generally not covered by insurance, though some patients with specific diagnoses may get partial coverage. Most hormone optimization clinics operate on cash-pay or membership models. HSA and FSA funds can often be used. Ask clinics about package pricing for longer-term programs.
Look for clinics that take a comprehensive approach. The best hormone clinics don't just check one or two hormones — they assess your full hormonal picture including sex hormones, thyroid, adrenal markers, and sometimes metabolic markers. This comprehensive approach produces better outcomes because hormones interact with each other.
Prioritize clinics that do thorough lab work and ongoing monitoring. Hormone levels change over time, and your protocol should be adjusted based on follow-up labs. Expect labs every 3–6 months once stable, more often during optimization. Clinics that prescribe hormones without follow-up labs are not providing adequate care.
Consider the clinic's experience with your specific needs. Women's hormone health has different considerations than men's, and perimenopausal patients have different needs than post-menopausal patients. Look for clinics with experience treating patients in your situation.
Finally, look for clinics that emphasize lifestyle alongside hormones. The best hormone clinics understand that hormones are one piece of the puzzle — nutrition, sleep, stress management, and exercise all affect hormonal health. Clinics that address the whole picture tend to produce better long-term outcomes. Use this directory to find and compare qualified hormone therapy clinics in your area.
